I have searched the web using Google and have found an article. I am sure that you can find the same. Have you conducted any literature survey on the topic and formulated the project's aim and objectives? The design of the questionnaire will depend upon the objectives. Have you got an organization in mind that will give you access? Does that organization use QCs?
