Actually, management has given me a chance to fill out a performance appraisal form. How should I fill it out? What things should I include in this form? Could you also send me any format or sample letter you may have?

Hi Paras,

I got the opportunity only twice, but they were good learnings.

1. Never use the word 'I', especially when you are in a team. Always include your colleagues/fellow members while describing your achievements. (What I did in the last appraisal)

2. Write only what you have actually done. Do not use language that makes it seem like you are just boasting about yourself. If you need training or need to learn something related to your job, then you must be frank about it. This honesty helps in better evaluation. If you had any shortcomings or failures, do not exaggerate them, but if necessary, do mention them.
