As far as I know, there is no prescribed form for surrendering a sub-code number according to the ESIC procedure.

Why do you want to surrender such a sub-code? Has the branch for which the sub-code was allotted been permanently closed? If so, you can inform the appropriate Regional/Sub-Regional Office and Branch Manager of ESIC that your branch has been closed. This is necessary to ensure that your branch is not treated as a defaulter when these offices do not receive any compliance regarding your branch.
