The Management of (such and such company) as an interdisciplinary platform of (the depatytment )has been guided by the need to address crucial policy issues associated with reporting pattern for the staff of the ____________. __________________________________________________ ______________ (Now address the issue)
If anybody in the ______(department name) will not follow the same he/she will be penalized by getting deducted ___days salary from their monthly remuneration.
Our association promotes teamwork in every ways to empower staff. We always believe in keeping the interest of each of its team members and we anticipate an atmosphere of trust, compatibility , commitment and punctuality among our team members.

This has reference to your leave application for ..... days during ..... to ....., which was approved by the management and you was supposed to join back your duties on ........
However, it is reported that you remained absent from work without leave on...... without prior sanction of leave or any intimation to this office.
In the past also, you remained absent from work without leave on ..... (insert the occasions when the employee had remained absent on similar occasion, if any).
You are directed to explain as to why action should not be taken against you as per the HR Manual for unauthorized absence within three days of issuance of this Memo. If you fail to satisfactorily explain the reasons for your unauthorised absence, action, as deemed fit and necessary, will be taken against you.

Whereas you proceeded on leave duly granted to you from .... to .... and whereas you have failed to report on duty on the expiry of the said leave and have thus, overstayed and remained absent from .... to ...... without obtaining prior sanction of leave and without even an application or intimation for grant of further extension of leave.

Now, therefore, you are hereby called upon to show cause as to why you should not be proceeded against under the Service Rules / Standing Orders.

The above act / omission on your part is of grave and serious nature and makes you liable for disciplinary action. You are therefore directed to submit your explanation within .... days from the receipt hereof. Should you fail to submit your explanation within the stipulated time, it will be deemed that you admit the charges and have no explanation to offer. Disciplinary action will therafter be taken without reference to you.
