You can simply address the letter to your superior, stating that you would like to resign from your current position due to personal reasons. Kindly follow the procedure regarding the notice period rule of your company.

Dear [Recipient's Name],
After giving considerable thought, I have decided to resign due to unavoidable personal issues within my family. These circumstances prevent me from continuing my services any longer.

Although it was a difficult decision to step away from such a pleasant working environment, there are times in life when we must prioritize family matters. It is with this in mind that I have chosen to tender my resignation after thorough consideration.

Please accept my resignation and process my release at your earliest convenience.

Warm Regards

Please follow the instructions of Rohit regarding the reasons that were outlined by him. Avoid mentioning specific or particular matters in your resignation letter. Providing a general understanding to the concerned Head of Department should be sufficient. The HOD will then forward this letter to the HR department for further processing. The HR department will handle the Full and Final settlement process.

Bashir
