In case you are seeking exemption from PF department, you should ask the regional PF office for the list of requisite department.

As rightly quoted, firstly, you have to make an application to the regional PF Commissioner for an exemption. Secondly, once you get the exemption, in consultation with the Regional PF Commissioner, contact eligible insurance companies for further processing.
