Hello Sir,

I need your help for PF Transfer Issue. When I left my previous company, I had asked to withdraw PF to my bank's account. However, due to some operational reasons, the account was closed by the bank. I had gone through the site [EPFO](http://epfindia.nic.in) on this point 22) discuss the same thing, and they have suggested submitting an application stating this problem. I had a discussion with the company HR about this; they are ready to provide such a letter but are asking for a "proforma." I have no idea what a "proforma" is!! Can anyone please let me know about "proforma" or provide me with a sample "proforma." I will be very grateful to you.

Dear Yogesh,

What you have to do is to submit a reauthorization form giving the corrected account number, and you will get your cheque. This is a common issue that occurs with software people who ignorantly provide the wrong salary account number, which becomes invalid once they stop receiving their salary. It is high time youngsters started using nationalized banks' savings accounts in the long run as they are reliable and do not unnecessarily close accounts.

The reauthorization form is available on the EPFO webpage. If not, you could ask for it in a reply, and I could try to attach the same.
