Hai Nilesh Just write a letter to the Bank Manager that you would like close your current account and mentioned the reason for closure. Thats all. MANOKAVIN

There is no standard format. The normal practice is that you need to make an application to the Branch Manager of the bank on your letterhead and get it signed by all the authorized signatories (this is in case the current account is not that of a proprietorship firm). You will also have to surrender unused cheque leaves.
