First need to activate your UAN as per your record , and after that you need to do the process for name change. If there is minor change you may change online by login employee portal and of there is major changes then need to submit the Joint Declaration form with supporting documents like Appointment letter, Salary Slip , ESIC No. ( IF esic member ) , AADHAR card copy.
