First, let me tell you one thing - IP cards are available on the ESIC website. You just need to inform the employer as soon as possible to provide the IP card, but remember that without the family photo card, it is not valid.
ESIC facilities are not available in private hospitals. If you need to make a claim, please give me a call, and I will guide you on the process.

First of all, please visit the dispensary office with all medical documents and get suggestions from the concerned employees with ESIC temporary card and form-37. Then submit one set of medical documents to the HR Department and ask them to submit the accident file to the ESIC local office.
There is no information available regarding whether family photos are required or not.
