1. Employee has a Permanent card from ESIC that has not been updated with the HR Department.
2. As HR was not aware of it, a new ID was created in ESIC.
3. The employee wishes to continue using the previous permanent card.

It is always better to have the earlier issued ESI card with earlier IP no so that he can continue to get the benefits of ESI.
