Please provide me with the termination letter format for a situation where an employee is terminated from the service due to absence from the organization without informing the concerned department or manager for more than 5 days.

Directly you cannot terminate for un authorized absenteeism for more than 5 days. for more than 10 days only you can terminate with proper charges by issuing chargesheet, enquiry and termination.
