Hi all, I would like to know if there should be a separate performance appraisal form for the team leader and his/her team members. We have an appraisal form that includes all the parameters necessary for HR to gauge the employees' performance over the year. Kindly respond as soon as possible.

Logically speaking, yes, there should be a separate criterion and form for the team leader. The team members contribute individually to a team, but it is the task of the team leader to harmonize, coordinate, and deliver the team target, however small the team may be. There may be some common attributes in the format for the team leader and members, but they cannot be the same 100%.
