I am in great need of a performance appraisal format for different departments such as:
1) Performance appraisal format for Accounts staff
2) Performance appraisal format for Technical/Design staff
3) Performance appraisal format for Sales staff, etc.
I have gone through the data but couldn't find separate formats for each department. Can anybody help me? It's very urgent.

You have to be clear on what you are looking for. I am quite sure you are not seeking a performance appraisal format for different departments because, in most organizations, there is typically only one performance appraisal form. Are you looking for the Key Performance Areas (KPAs) and Key Performance Indicators (KPIs) for different departments?
