I cannot send the filled challans or returns. If you have any doubts about these forms, I can clarify them for you.
Furthermore, the last date for monthly contributions is the 21st of the following month.
There will be two half-yearly returns: one for the period ending on September 30th (April-September) and another for the period ending on March 31st (October-March). The due dates for these are November 11th and May 12th, respectively.
An annual return in Form-01(A) must be submitted every year before January 31st.
Currently, ESI is to be paid on all components, i.e., BASIC, DA, and all allowances (except Washing Allowance).

What needs to be done apart from the above-mentioned intimations, filling out Form 16, and submitting it to the nearest and applicable dispensary and ESIC office (regional)? In return, we may receive a certificate of fitness given to the insured person (IP) by a doctor, which is to be sent to the regional office of ESIC.
How long will it take to make the payment to the person who has claimed those benefits? Please provide an idea of the timeline along with the percentage or amount, or how much compensation the IP would receive at the end of this procedure.

Dear Seniors, One of our old employees got into a road accident on the way to her hometown (Kanpur) and she needs to be hospitalized in the ESIC hospital of Kanpur. However, the ESIC staff is not ready to admit her due to the requirement of some paperwork. Can any respected senior help me out with this matter urgently so that I can assist her from our office? Utmost at the dispensary, they may ask for ESIC-37, i.e., Certificate of Continuous Employment. In case of an emergency, they may ask for the Eligibility Certificate from the concerned branch manager. Nowadays, due to computerization, the status of both can be viewed using the IP portal, which is available on the ESIC portal under the caption "IP Portal" at the bottom left corner of the website homepage of www.esic.in. However, this can only work on Mozilla Firefox.
In your case, I strongly feel that they may require ESIC-37 and the employee's Permanent Card. If not available, a Temporary Card may suffice.
Wishing you all the best. One more small suggestion: Every HR should have a list of ESI dispensaries, hospitals, and private tie-up super speciality hospitals within their state.

You may submit the said Accident Report even now along with reasons for such delay. Though the submission of the accident report late is a violation of the Act, there is no alternative at your level. If the requirement is not complied with, the insured persons will not receive the benefits to which they may be entitled.
