I am very thankful if you could provide me with details from a clear perspective regarding ESI, as there is no proper process for ESI nomination form submission and number allotment to the employees.

Kindly let me know about the following aspects:

1. How to submit the ESI nomination form and what enclosures the employer needs to submit to the ESI office along with the form (e.g., family photo, parent declaration form, etc.).
2. After submission of forms, what is the time limit for getting ESI cards?
3. Please suggest if there are any new procedures evolved for ESI.
4. Can anybody explain about the Smart card for ESI?
5. After getting the card, how can we get medical benefits, and whom should we approach for proper treatment?
6. How can we find out what type of medical benefits employees can avail of?
7. Please suggest which website is good for knowing the latest updates on ESI.
