Dear Sir,

Brief facts are as under:

1) A driver on our payroll was remained absent from duty from 09/11/2020 to 31/12/2020. We have issued warning memo to him on 08/12/2020

2) By way of letter dated 21/12/2020, he tendered apology and joined duty on 01/01/2021.

3) Again he was absent on duty from 05/06/2021. We have once again issued warning memo to him. He replied to warning memo and stated that his health and his mother's health is not well. But when our representative visited his residence, the driver was drunken and not in a position to talk with our representative.

4) On 06/07/2021, we once again sent him final warning memo. Same is duly acknowledged by him. Thereafter, he neither joined the duty nor replied to final warning memo and till date he is absent on duty.

Kindly provide the draft Show Cause Notice on the aforesaid facts.
