Dear Mr. Vinay,
It would be really great if you could please help me in preparing a formal payslip.
Details: For example, if the gross salary of an employee is 21,500 Rs, can you provide me with the salary structure for this amount? This includes how to calculate Basic, HRA, PF, PT, and other necessary allowances. Additionally, could you please give me a brief explanation of Payroll?

The review sheet looks quite professional. I am not sure about the allocation of points. I have a couple of queries to share. Whenever a trait has to be rated, it has to be defined. If you have to rate problem-solving ability on a scale of 1-10, you will have to explain to the rater which response corresponds to which rating. Secondly, it becomes tough to decide on the rating as the scale increases. I would suggest using 2 dimensions: demonstrated or not demonstrated, in place of a 10-point scale.
