Can some one give me a format of email to send to the employee who is taking excessive breaks a day at work?

Before sending him an email, have you given him one-on-one feedback in private? By the way, do you have records of his excessive breaks? Where does that employee spend the time during extended break times? How many instances of extended breaks are there? An email should follow personal feedback. Direct emails may leave that employee demotivated.

Is this an isolated case, or are there others who also take extended breaks? Are you from HR, or are you the manager of that employee? Has the manager given feedback? Have you trained your managers on "How to Give Feedback"?

You have asked for the format of the email, and in return, I have asked many questions. However, unless we know the background of the case, it may not be possible to give you an accurate solution.
