Document Submission for ESIC and PF After Employee's Accident
One of my employees got into an accident and passed away while he was on his way to the office. He was covered under ESIC. What type of documents should be submitted to ESIC and PF?

Please prepare an accident report in the ESI Accident notification system and attach a copy of the FIR, the Hospital Treatment Report, and the Funeral certificate. For any further assistance, please contact your local ESIC office.
Regarding PF, the nominee can raise a claim as usual in the event of a normal death.

As per ESIC, the nominee will receive ₹5000 as funeral expenses only. In the case of EPF, the nominee and the minor dependents will receive the monthly pension. To avail of this benefit, you need to submit the required documents such as 10-D, Form 5-IF, etc.

You also need to fill in the form for the Funeral benefit. The amount is Rs. 10,000, which the family of the deceased will receive from ESIC. Furthermore, his family will also receive Rs. 1,30,000 from EDLI as you contribute to his PF.

Regarding ESI Form-16, it's an accident report that should be submitted to the Chief Inspector of Factories and another to the insured medical officer concerned.
Form-25A for Claiming Funeral Expenses
Form-25A for claiming funeral expenses should be submitted with the death certificate/declarations duly attested within 3 months from the date of death.

Dear, clement, I think EDLI benefit is not fixed at 1,30,000/- but it depends upon the service and epf deposited on behalf of that particular employee.

1. Should the employee coming to the office use company transport to receive all the benefits, or if they come by another mode, will they still get all the benefits? Is the coverage of ESIC only at the working premises or at all times and places?
2. Should the accident report be sent to both the ESIC office and the factory inspector?
3. ESIC provides ₹5,000 for funeral expenses, but what is the ₹10,000 for? I don't believe that EDLI is fixed at ₹1,30,000 as mentioned earlier.
Please clarify my doubts.

Please go through the attached extract from the ESI Act and note the highlighted portions for your guidance. There are numerous case laws on the subject that are very precise regarding the circumstances you referred to here. It would be beneficial to refer to them as well to have a clear understanding of the context and guidance. If you are unable to find them, please let me know.
Regarding PF, there are a few procedures that have already been indicated by many.
For other benefits, etc., it would be advisable to refer to the Workmen's Compensation Act as well, which would be an added advantage.
First and Foremost:
i) Police FIR certified copy,
ii) Post-mortem report,
iii) Death certificate issued by the local administration, and
iv) Collect copies of nominations filed by the deceased under ESI, PF, Gratuity Act, etc.
v) File papers for obtaining a 'Succession Certificate' in favor of the deceased's nominee/wife as the case may be at the appropriate court.
vi) In order to smoothen the process, obtain a "NOC" from the close relatives such as parents, brothers, sisters, or any other dependents (if any).
Funeral Expense enhanced from Rs. 5,000 to Rs. 10,000 w.e.f. 1.4.2011 vide notification No. R.15/13/2000-Bft.II dt.19.5.11 issued by the Joint Director (BFT), ESI, New Delhi.
