Employee Details Collection
Name, Date of Birth (DOB), Age, Sex, Marital Status, Family Details, Address (temporary, permanent), PAN Card, Passport Number, Previous Employment Details, Medical Record (any special remarks, including Blood Group - which is important), Emergency Contact Numbers.
You can collect all these details in an Excel sheet and keep it updated as per your needs. Hope this is sufficient.

In addition to what Ravi said, include the qualification details, nominee details, etc. in a spreadsheet. You can create such a format; it is not a difficult task to create a spreadsheet.
