First of all, you need to create different departments, and based on that, you can create the performance forms. You can include the following information in the respective forms:

Employee Name:
Designation
Department:
Date of Joining:
Due Confirmation Date:
Review Date (when it was actually done)
Name of the Appraiser:
Designation of the Appraiser

At the bottom of all the pages:
Signature of the Reporting Authority:
Signature of the Appraiser:
Signature of the Appraisee:
Signature of the HR:
Date:
