1. I need to know what forms/registers need to be maintained.

1. Sir, in my opinion, your organization must examine the claims for maternity benefits submitted by the female employees of your establishment with reference to the law laid down under the Maternity Benefit Act, 1961.

2. For the procedure of such payment of benefits and maintenance of records, I think there must be some State Govt. Rules of the state in which your establishment is located. You can get the forms and registers from the said State Govt Rules.

3. Please also see if your establishment is covered under the ESI Act, 1948, or not? If so, whether the claimant female employee is covered under the ESI Act?
