Dear Dhrt,
First of all it is largely a state subject. (rules of Gujarat). Yet the minimum in 9 days of national and festival holidays you have to declare. Normally Form XVII speaks of details of salary. Buy the local shops and establishment Act book with rules as also the state rules of Factories Act. That would guide you better.
