PF Withdrawal Forms Processing: Queries and Solutions

1. In Form-19/Form-10C, we need to mention the family details of the employee (like member name, date of birth, relation with the employee) since the employee has completed more than 10 years of service with the company. Now, this employee is a village native and is not aware of the date of birth of the family members. He does not have any document to show his family members' date of birth. At the time of his pension, there may be a problem regarding the date of birth. How can I tackle this situation here? Please suggest.

2. One of my acquaintances told me that only a few banks accept the pension amount and then transfer it to the concerned employee's bank account. This employee has his bank account in the Bank of Maharashtra. So, what is the possibility of getting his pension amount in this bank account?

Pls find a pdf file containing the entire SOP of EPFO reg. disbursement of Pension. The list of banks are also mentioned therein. Hope it helps.

Solution for Date of Birth Issue

Regarding the date of birth issue, you may take an affidavit from him mentioning whatever date of birth he gives. It should also mention that the affidavit is sworn to show the dates of birth because there is no other documentary proof. This will help in times of need.

In the personnel file of the employee, the employee's date of birth is available, but not that of his family members. So, while processing his PF and pension withdrawal forms, there may be a problem concerning the date of birth of his family members.

Yes, unfortunately, details were not maintained properly earlier. That's why we are facing this problem.

The link pasted above is intended for an employee to authenticate his Date of Birth in the absence of any written/approved document. Since we also require the Date of Birth of his family members, I have requested the employee to include his family members' Date of Birth in the same format. Will that be okay? Please provide further comments on this.

Provident Fund Date of Birth Documentation

Usually, the PF office does not insist on proof of date of birth for provident fund purposes. They may request it specifically in cases related to pension, especially for children. My suggestion is to submit the forms with the details as provided by the employee initially. If the forms are returned, the employee can then be advised to provide any of the following documents as proof of date of birth:

1. Certificate from school
2. Certificate from the registrar of births
3. ESI record
4. Certificate from municipal authorities
5. Any other document suggested by EPFO officials

The affidavit format can be used wherever a documentary proof for date of birth is not available. Hence, it should meet your requirement.
