My first question is whether the amount accumulated in EPS can be withdrawn by filling out the EPS-95 Form 10c separately in the Commissioner's office or not.

If your PF and EPS are managed by the EPFO, both numbers will be the same. However, for exempted trusts, this may vary. In such cases, if you know the PF number, the corresponding number can be obtained from the RPFC.
Since you have only 5.8 years of service in EPS, you are not eligible for a pension. Nevertheless, you can apply for the withdrawal benefit through Form 10C. The 5.8 years will be rounded up to 6 years, and the corresponding factor for 6 years in Table D is 6.07. Consequently, you are eligible to receive the average salary of the last twelve months multiplied by 6.07 for EPS withdrawal.
In your situation, it is recommended to apply for a scheme certificate, which is also done through Form 10C, so that this service may be combined with any future service. If there is no future service, the scheme certificate can be used for the withdrawal benefit.
