Dear Friends, I am filling out Form 10-C and Form 19 to withdraw my Provident Fund from my last job. Do any of you have experience with this? Form 19 is okay, but in Form No. 10-C (EPS) (Employee's Pension Scheme, 1995), I do not know what to fill in Serial Number 5. What details do I need to provide for this?

Code No. & Account No. | Region | SRO | Estt. Code | A/c No.

Understanding EPF Account Number Structure

The EPF account number has three main parts: Region Code No., Sub-Region Code (optional), Establishment Code, and the member's account number. In the form, Region Code No. stands for the code of the Region. For example, for Kerala, it is KR, followed by the Sub-Regional Code, such as KK for Kozhikode or KNR for Kannur. This is followed by the establishment code number, like 12365, and the member's account number, such as 123....

Dear Madhu, thank you for your help. I received the PF number from my last job, which is MP/15776/196. I believe MP is the region code number, 15776 is the establishment code number, and 196 is my account number. If I am correct, it should be structured in this way. I am unsure about the sub-region code.

Question About Form 19

My other question pertains to Form 19, serial number 4. There is another account number listed; is this the same as MP/15776/196?

Yes, the sub-region code is not always written. Therefore, you should write only "MP" for the region code, "15776" for the establishment code, and "196" for the member's account number. In Form 10C, it should be written against question No. 5, and in Form 19, against serial No. 4.
