I have uploaded the skill set and behavioral assessment sheet. Please let me know if it is complete, or if it requires any additions/deletions of skills or behavioral indices. I am planning to integrate the assessment with the performance appraisal form and also provide a graphical presentation of their performance.

u can add more soft skill column in that like Team Player, Ready to take ownership, assertice etc if you think it suits to your requirement you can add this
