If you are a student doing a project, you are supposed to do a literature survey. Have you done it? If so, have you prepared the aim and objectives of your study? The best way to find a questionnaire is to conduct a literature survey and then adapt one from what others have used. Please see my posts on Research Methodology and MBA Projects at CiteHr.
