I'm looking for symptoms of low-performing employees, such as:
1. Not delivering on time.
2. Low morale.
3. Not being a good team player.
4. Frequently taking leave.
5. Showing low project commitments, etc.
Is there a list available out there? I did a quick search without much luck. Any pointers are appreciated. Thanks.

Just thinking... no pro-activity, no willingness to help out colleagues, sick leave rates, find always reasons why something won’t work, ...
You HR have just become sick...
How the hell can you term an employee as a low performer if he takes more leave, or does not help colleagues? An employee may have many personal reasons to take leave, and if you can't understand that, then why the hell did you take the HR job? Do you yourself have a good rapport with your employees? I'm sure you don't, and neither do you help your employees, so it is assumed that all HR are low performers who just sit in the office and while away time.
Come on, get away from this sickness, and stop keeping a 24-hour watch on employees. Employees do perform; just leave them to their own way, and don't think that you're some excellent performer. 😡😡

If you have to do 100% of the job in 30 days but you end up taking 20 days off (for whatever reasons), how much % of the job would you have completed in 10 days? 33.3%... I think that's 'Low Performance' by any standards.
You don't work individually in any position. You HAVE to be a team player. Your attitude is more important than your aptitude. If you can't help your teammates, then that means there is something wrong with your attitude. That also amounts to 'Low Performance'.
And what exactly do you mean by 'Understanding Employees'? Giving you a higher rating irrespective of your performance just because of your 'personal problems'? Is that 'Understanding' to you?
I think not. For such a low rating, under normal circumstances, you have to terminate an employee. But instead, if HR has given you a low rating and kept you onboard, that's because it 'Understood' that your low performance is because of your 'personal problems' and hopes that you improve your performance next year.
I think it's high time you quit complaining and whining about HR.
