i have made letters for the same. formats should have the follwong;this is common for both contract ltrs as well as employee appointment ltrs.
department, location, salary details, reporting authority
in contract letters you need to specify the tenure of contract. also theirs is a fixed salary.
for employee appoinment ltrs you cud mention teh ctc or attach the salary structure at the end of teh letter.

You will have to do the necesarry changes.
Please put down the start date, end date date and the terms of contract as an annexure to this appoinment letter.
