I have an MBA, and I would like to do a Ph.D., but I am confused about the topics I am interested in: leadership, organizational culture, managing change, organizational performance, and performance appraisal. I think they are all related, but I cannot settle on an exact topic area. Can anyone help, please?

To choose a PhD topic, first choose one of the above areas that you are most passionate about. A PhD is a long process, and you have to make sure you really are passionate about the topic you choose because otherwise, you will be tempted to give up. Then start reading about that topic and start identifying areas that are under-explored. A PhD should make an original contribution, so you need to identify gaps in knowledge in the area you are researching. Start broadly, then zoom into where your interests lie.
Hope this helps as a start.
