Your questionnaire will be based on the training you have done in your organization. We do not work for your organization, so we cannot devise the questions for you. If you are looking at the effectiveness of training overall, then you need to talk to the various divisions in your organization and determine if the training has changed anything. Is the company more profitable from training staff to do jobs better or more efficiently? Has training improved the overall loss of downtime due to accidents? Is there better morale and teamwork, etc.? Only you can devise the questions that need to be asked based on your particular organization.
