Respected seniors, we have recently submitted the ESIC half-yearly return of contributions to the local branch office. We have only 30 employees who are covered under ESIC; hence, we do not require a "Certificate by CA." However, the ESIC office has stamped "SELF CERTIFICATE NOT APPENDED" on our acknowledgment copy of the return. We are submitting the R.C. in the revised format as per the notification issued on the 14th of March 2008. Please help with the format of the "SELF CERTIFICATE" that we need to append. There is no format on the ESIC website where we prepare the online return of contributions. Please help.
