Dear Sir/Madam,
I have been working in a solvent plant for 14 years, and last year I submitted a letter to my MD (Letter is attached). After reading the letter, he fixed my salary at Rs. 8000 per month starting from December 2009. This year, in April 2010, he increased the salary of all staff members except mine. When I inquired about this, he mentioned that my salary had already been increased. In response, I explained that the raise was for the period of 2009-2010 and that I am eligible for an increment this year. However, he has not taken any action in this regard. Therefore, I kindly request your advice and assistance in drafting a letter requesting my increment. I would be grateful for your help.
Thank you.

I think you need to concentrate more (in the later) on your achievements and not so much on your personal expenses. Try to add more details abou the 2 job duties that you are handling. Good luck.

Dear Mr. Anupam,
I appreciate your boss who has increased your salary based on your letter, and it was increased as you wanted. However, you should never pressure your boss to increase it always. You should concentrate on your work, and your work should impress your boss, rather than your personal problems.
Hope you will take it in the right spirit.

Dear Mr. Anupam,
After reading your letter, I had two doubts:
1) The letter is written in Jan 2009, then you got an increment in Dec 2009. Why did your boss take more than eleven months to consider your request?
2) In your letter, you specifically requested an increase in your salary up to Rs 8000. He has done the same thing within a span of eight months. What circumstances have changed in your personal life?
Moreover, any management cannot increase the salary for personal expenses. An increase in salary depends upon our commitment and effective work done in achieving the organizational goals.
Thanks and all the best. Do not think about your personal expenses; it might affect your career.
