At the very outset, kindly excuse me for taking your precious time to read my request in your busy schedule.
As you are kindly aware, sir, recently I had undergone (treatment).
While I was discharging my duties in (location), I had felt uneasiness and giddiness, and the local doctors advised me to go to a hospital with better facilities.
Accordingly, I rushed to (hospital name), and after investigation, I was shifted to (another hospital), for emergency treatment.
The specialist doctors who attended to me have given lots of dos and don'ts to avoid further damage to my health. Some of the important ones are as follows:
1. Take the medicines as prescribed and on time.
2. Follow a bland diet without spicy, oily, or saturated foods.
3. Avoid anxiety, stress, and strain.
4. Refrain from lifting heavy objects.
5. Avoid climbing to higher altitudes.
The current conditions at the site do not allow me to follow these precautions. Due to family responsibilities and work pressure, I am unable to adhere to the doctors' advice. This situation has caused a lot of mental distress to my family, and the lack of nearby emergency medical facilities is also concerning.
Therefore, I humbly request you to arrange a transfer to another location so that I can receive regular medical checkups within our organization.
My family and I would be forever grateful for your assistance.
