In order to help people decide whether they would like to participate, you need to give more information about yourself, whom you represent, and what the purpose of the survey is. Also, it is a bit much to expect us to cut and paste your survey questions and then save it and send it to you. Instead of typing the questionnaire, had you posted the same as an attachment, it would have helped.
