Hi, you just did an appraisal review and used these parameters. I will provide you with an outline:
a. Involvement in work
b. Relationship with coworkers
c. Achievements
d. Delegation
e. Motivation
f. Skill sets
You should also add some other criteria as per what I know. Sometimes this might be wrong, so excuse me.

Dear HR,
The parameters to be used for selecting Employee of the Month/Quarter/Year should be built around the "values" of your company. If your company does not have value statements, then base your selection on the desired soft skills and competencies for your company. This would help you to reward the desired behaviors and reinforce the same across the organization.
