Could you use the SA-8000 standard, as well as current concepts in corporate social responsibility, as the basis for the areas/questions you would want to include when you craft your questionnaire? This is a topic that I have increasing interest in, but none of my current research or clients have presented the opportunity to build this yet.

I am trained in SA 8000, the Ethical Trading Initiative, and worldwide Responsible Apparel Production. I have worked closely with various companies engaged in the manufacturing sector. If you want to conduct a real survey, the situation is dismal in several sectors. Ask any labor inspector or trade union member, and they will give you the true story. However, I have seen worker interviews where workers are dismissed en masse the next day.
Asking a worker's family member, "What is the future of your kids?" would reveal all the details if posed by an NGO. Let me know what I can assist you with.
