The location is suitable to my health or nearer to my home. (If there is change in location)
You may choose either or mix suitably.

Just say "Personal Reasons". I have tried in three to four times.
Or for better opportunity and prospects, good environments.
Or say frankly, since two years I haven't gotten any increment, promotion, benefit etc.

yeah correct but how can we convince the new company’s HR that we r leaving of personnel problems and to explore for other opportunities????????
