Hello Sal,

To eradicate the turnover issue, I would suggest you that immediately prepare an employee satisfaction survey with quite an open objective i.e. retention and improvement. Conduct the survey within the organization ASAP. Against each factor ask the employees to give their input and try to keep the survey forms anonymous for an objective overview and diagnosing the real causes for the said issue after the survey is conducted prepare a report on the basis of collated data.

Keep the drop box (comment box) in every department for suggestions and pay heed on the suggestions while implementing on the good suggestions improve the areas in the prevalent and mention that due to suggestion from the employees the such and such changed has been brought in for betterment and improvement of organizational culture.

Conduct reviews with every departmental head regarding the issue and prepare a report on the basis of those reviews (while pointing out factors for the cause either less salary issue or extra job responsibility and low pay or coaxing by the seniors, compensation plan etc.) and send the report to the top Management mentioning the areas and suggestions for improvement.

Streamline your process for conducting the exit interview and do conduct the exit interviews.

Prepare an exit interview form and try to keep all the factors that could possibly affect in.

Redefine Job roles, Job description as well.

The above are the immediate measures that you can do to diagnose and eradicate the turnover issue.
