The questionnaire is pretty good. For your general idea, kindly organize yourself on what data is to be captured to study the perception of the employees. For example:
1. The profile and role of the employee in the organization.
2. About the relationship with his peers, subordinates, and superiors.
3. About the organization as a whole and its departments. Study the linkage between the department to which the employee belongs and other departments, as well as his perception about the functioning of the departments.
4. Try to include more options than just using yes/no types. You can use 5 rating scales such as dissatisfied/highly dissatisfied/neutral/satisfied/highly satisfied.
5. Combine a few questions into a single question with sub-options so that when you convert the question into a chart, you will be able to easily relate/correlate things.
Make sure you include a few questions that would enable you to conduct a correlation study, for example, the correlation between the employee's workplace and his perception of the role/responsibilities.
Take a pilot test of the questionnaire with your friends, and maybe you can get a few suggestions from them since the questionnaire is objective for you but subjective for them.

I am only doing a one-month training in the company, and half of the time has already passed. I will be in the company until 7th Feb only, so it is urgent for me to complete the study. The questionnaire was designed with the help of the AGM HR. Now, he wants to delve more into it, but due to time constraints for me, he has suggested finalizing it as is. Another problem has arisen in front of me: correlating the findings with motivational theories testing and, if possible, some other aspects. I am not able to understand how to do this as it's my first time. I have studied motivational theories, so for now, I think only the Herzberg theory can be applied based on this questionnaire.
I am unsure about what else can be correlated. Please guide me as it is much needed.

Correlation is a tool used to relate between two factors. Apart from Herzberg's theory, I hope you can also incorporate Maslow's Need Hierarchy theory into your questionnaire. There are a few questions in your questionnaire that explore environmental factors. Why don't you relate these to the Basic Needs of the need hierarchy?
Identify the questions that can be related to all five needs, but ensure you do not deviate from the primary objective of assessing employee perception and avoid being biased towards motivation theories. You can explore the connection between the experience with policies and systems question (Q No. 17) or with Q No. 21 or 29.
Free your mind and discover which questions can be interconnected. I believe you will find many of the questions are interrelated, leading you to a solution.

Thanks for the reply and giving me the motivation to work on. Right now, I am conducting a survey, but the worst part is that I am not able to interact with people because they are too busy with their work or only seem to be busy. This is a public-private company, and no one is eager to work here.
Now, I am in a dilemma of how to get responses from these employees. The worst part is that my mentor has been transferred to another department, and he is no longer the HR head. I am getting sick of this.

I hope this data collection stage would be a tough situation in a project. Only if the sample size is more, I think you can use many of the data interpretation tools. Try collecting as much data as required and as possible. You can ask people about the situation and request them to complete the questionnaire. If there are any training sessions happening in the company, you can ask the trainers for help and maybe distribute the questionnaire and have it filled there.
