You have not stated the purpose of the survey. The survey can be conducted in two major ways, depending on the desired outcome.

If the purpose is to use the survey data for making long-term policies, then it is advised to follow Gallup's theory. You can find more information in "Social Research Methodology" by Dr. P. V. Young.

If you need an immediate solution for a specific problem or to improve existing behavior, in that case, I would advise you to follow the Organizational Development (OD) approach. This involves conducting the survey through Behaviorally Anchored Rating Scales (BARS) and planning immediate interventions.
