Hi Kailash,
You have not stated the purpose of the survey. The survey can be conducted in two major ways, depending on the desired outcome.
If the purpose is to use the survey data for making long-term policies, then it is advised to follow Gallup's theory. You can find more information in "Social Research Methodology" by Dr. P. V. Young.
If you need an immediate solution for a specific problem or to improve existing behavior, in that case, I would advise you to follow the Organizational Development (OD) approach. This involves conducting the survey through Behaviorally Anchored Rating Scales (BARS) and planning immediate interventions.
If you need further assistance, feel free to write to me at urmanas@yahoo.co.in.

Employee opinion surveys allow an organization to understand employee perceptions. Perception is reality. Because employees at every organization act on the basis of their perceptions, management must be keenly aware of employees' views. Employee opinion surveys deliver a successful means of measuring and acting upon employees' current beliefs on many job-related subjects.
CRITICAL FACTORS TO BUILDING A STRONG ORGANIZATION, WHICH COULD BE USED IN EMPLOYEE PERCEPTION SURVEY, ARE:
* COMMUNICATION
* EMPLOYEE EMPOWERMENT
* QUALITY IMPROVEMENT
* CONTINUOUS IMPROVEMENT
* JOB SECURITY
* JOB STABILITY
* MANAGEMENT COOPERATION
* SATISFACTION WITH THE COMPANY
* PHYSICAL WORKING CONDITIONS
* ORGANIZATION AND EMPLOYEE DILIGENCE
* REWARDS / RECOGNITION
* EMPLOYEE TRAINING / DEVELOPMENT
* RESPECT / TREATMENT OF EMPLOYEES
* TEAMWORK / COOPERATION
* SATISFACTION WITH THE JOB
* STRESS / WORKLOAD
You should tailor the question based on:
- your current organization needs
- your expected outcome for this exercise.
Dimensions
Each item in the employee opinion survey could be used to measure the following key aspects of employee satisfaction:
- Overall Job Satisfaction
- Satisfaction with the Work
- Coworker Performance/Cooperation
- Pay Satisfaction
- Benefits Satisfaction
- Promotions/Career Advancement
- Supervisory Consideration
- Supervisory Promotion of Teamwork and Participation
- Supervisory Instruction/Guidance
- Communication
- Human Resources/Personnel Policies
- Concern for Employees
- Productivity/Efficiency
- Training & Development
- Physical Working Conditions
- Customer Service
- Strategy/Mission
- Job Stress
Benefits of conducting an Employee perception Survey:
One aspect constantly distinguishes top-performing organizations from the rest of the pack: motivated and satisfied employees. Organizations at which employees exhibit elevated levels of satisfaction enjoy higher quality products and services, as well as increased customer satisfaction and heightened overall business performance.
Implementing an Employee Perception Survey can be used to gauge current levels of satisfaction and identify opportunities for improvement as perceived by employees. Organizations choosing to utilize Employee Perception Surveys reap myriad benefits as they are able to:
- Learn about organizational strengths
- Identify key retention factors
- Discover opportunities for improvement
- Improve morale = reduce turnover
- Learn about potential cost-savings
- Enhance productivity/efficiency
- Use tools for positive change
- Value employees' opinions
- Utilize employees' recommendations and solutions
- Identify cost-saving opportunities
- Evaluate opportunities for improving customer service
- Streamline communication
- Learn about the perception of different management styles
- Use HR Solutions' National Employee Satisfaction Normative Database for Statistical Benchmarking
- Measure the effectiveness of the organization's programs
- Strengthen supervision
- Assess training needs
- Curb absenteeism
- Identify other areas for improvement
- Increase credibility of management
FOR THE SUCCESSFUL CONDUCT OF THE SURVEY, THE QUESTIONNAIRE MUST BE TAILORED:
- TO THE OUTCOME
- TO THE CULTURE OF THE ORGANIZATION
- TO THE EXPECTATION OF THE MANAGEMENT.
