Could you please provide information on the form that needs to be submitted and the other required documents? Additionally, what records should the company maintain for an employee who has passed away, such as a resignation letter? Your prompt advice on this matter would be greatly appreciated.

Eligibility for EDLI and ESIC Benefits

One of our employees, a driver, worked until September 11, 2016. From September 12, 2016, onwards, he stopped coming to work due to an argument with the night security personnel on the 11th. His last PF contribution was paid in October 2016, and he has not submitted a resignation letter to date. Last Sunday, he was involved in an accident, and on Wednesday, he passed away. It appears that his PF contributions have not been paid since November. Will he be eligible for EDLI?

Also, please advise on any benefits he may receive from ESIC.

Please guide me in this matter and take necessary actions.
