Forms Required for PF Submission in Case of Employee Death

Following are the forms you need to submit for PF in case of the death of an employee:
- FORM 20
- FORM 5IF
- FORM SFC
- FORM 10D
- FORM 5 and 10
- FORM 3A
- Original Death Certificate
- Nationalised Bank Account details of the Nominee
- Passport Size Photos

Form No. 20 - for PF withdrawal by Nominee, Form 10 D - for the benefit of the pension of the wife/children up to 2 Nos. EDLI: for insurance benefits under the EDLI scheme Form 5IF.

Documents required:
Death certificate issued by District Authorities. If nomination is not done by the employee, then forms should be supported by an affidavit of the nominee/legal heirs. Bank savings passbook of the wife and eligible children (of the same branch of SBI/Punjab National Bank or any bank authorized for pension purposes by PF authorities), 3 sets of passport-size photographs of each eligible member under the pension scheme, family details on the company letterhead, Date of birth certificate (in the case of children issued by the school authorities), Certificate of the company declaring that the employee was in service of the company.

One of our employees left one year ago after eight months of service. Subsequently, he passed away without withdrawing his provident fund. At the time of his death, he was unmarried.

Can anybody help me with whether his nominee is entitled to a pension or not?

Arun Kumar is right, but here are some more things that are also needed:

1. Birth certificate of the deceased person because the PF authority wants to confirm that the deceased person was below 58 years old when he died.
2. Ration card copy of the deceased person's wife and children.
3. Birth certificate of the deceased person's children.
4. Bank account numbers of the deceased person's wife and children separately in State Bank of India, Punjab National Bank, or HDFC Bank Ltd.

Required Documents for PF Withdrawal Upon Death

In the case of a death, you need to file the following forms with supporting documents:

1. Form 20
2. Form 5 IF
3. Form 10 D in three sets
4. Form 5 & 10
5. Death Certificate
6. Date of birth certificate for each claimant, including the member as well
7. Bank Account for each member (in case of a minor under guardianship A/C). For this purpose, a bank account will be opened in a specified bank (SBI, UTI Axis Bank, Punjab National Bank, HDFC Bank)
8. Passport size photo, 3 copies for each member
9. Family photograph of all claimants

Note: In the case of a minor claimant, the major employee will sign his/her Form 10D on behalf of the minor.

Enrollment Certificate Form 5 IF

Certified that the member died on ____________________________ while in service.

Certified that the provident fund accumulations of the deceased employee Late Shri/Smt. ____________________________ A/c. No. __________________ were paid to Shri/Smt./Kumari ____________________
