Hi,

I need a letter format for Temporary Break-up in employment due to illness (Medical Treatment) for the short term. The details are as follows:

1) Employee is on confirmed from 2 months.
2) As per the medical report, he has been advised medical treatment for 6 months for an infectious disease as it will be harmful if he is present on the premises.
3) He can join our firm after treatment completion as per the medical officer's certificate.

Please help me.
