I would like your tips, suggestions, and, if possible, templates to design a questionnaire that would be circulated to the employees to get their feedback on the existing system of handling HR-related work and setting up a new HR practice and department. Please help me out.

Hi Padma,
I was also thinking about it. I have taken up an HR assignment for a new company, and probably, a streamlined approach could only be achieved with a new system design. I have outlined the design to cover the following areas:
1) Legal registrations and formalities coverage
2) Resource planning
3) Relationship management
4) Compensation structure design
5) Queries management and grievances handling
6) Performance management
7) Pre/post-exit formalities handling
It would be highly appreciated if anyone could suggest ideas on areas where we should focus, and a questionnaire could really benefit all HR professionals.

I do not understand how a job description questionnaire will help in setting up the HR Department. For example, at http://en.allexperts.com/q/Human-Res...artment-HR.htm, Leo Lingham, who contributes very well to citehr, has answered such a question.
