We have lost our ESIC challans for July 09 and now would like to apply to SBI for a bank certificate with our bank cheque details. Can anyone guide me with the letter format at ?

Hi, MadhuraParab,

Have you lost just the challan of a month for ESI? Then you can easily apply for it. Just prepare a fresh challan of the same month and take it to the SBI officer and request him to acknowledge on the challan for that month since you had lost it. They will acknowledge over the challan you are producing and return it to you. We usually follow this way of retrieving our lost challans, but if the banker demands a letter mandatorily, then you can simply write a request letter for issuing a copy of the challan pertaining to the specific month and get it signed by the employer and submit it. You can clarify such simple issues with the banker itself. Since you had already remitted the contributions, it would definitely reflect on your bank statement, being the proof of your remittance.
