It is not so easy to describe all the functions a person performed during their service in a service certificate. Nowadays, many companies issue service certificates in the following format:

1. NAME
2. FATHER'S / HUSBAND'S NAME
3. DESIGNATION
4. DATE OF JOINING
5. DATE OF LEAVING

[SIGNATURE OF THE ISSUING AUTHORITY]

This format is commonly used in current practices.
