I am attaching the Training Feedback Form format, which may be very useful to you. When interviewing senior and middle-level candidates, try to prepare an interview questionnaire and confidently ask them those questions. However, avoid treating them as if you are interviewing a fresher.

I have a question on this: I noticed that the feedback form asked for name and department. In my experience in the USA and here in the UAE, that info has been optional with the intent to get more objective feedback without concern for being identified for the content.
Is it common in India to request your name, etc.? Why?
